Hi there, the reason I used the word 'accident' on this occasion is because the company is quoted verbatim. Afer re-reading I see the quote marks were missed in the strapline - this has now been edited, my apologies for the confusion. 

You're correct that we don't know how or why the incident happened, as we haven't received any further details.

Hello - you have used the word "accident" several times in this article. Was it actually an avoidable incident, brought about by rushing the bikes onto the streets before adequate risk assessments and other safety checks had been carried out? I wouldn't repeat what the company called it without proof that it was just - oops - an "accident"?
